Release Form
All youth who participate in the Youth Ministry Program at Saints
Peter and Paul Parish will be required to have on file a Multi-Media
Release Form. Sometimes a reporter may do coverage at one of our
activities or photos might be taken for use on our web page. The
release form indicates to us whether or not names, faces, etc.
of the teens may be used. Parental permission needs to be given
in order for us to send information regarding the Youth Ministry
Program via e-mail. This form will remain on file all year.

Sponsor Information Form
he choice of a sponsor for Confirmation is a very important one.
At Baptism your Godparents were witnesses to your belonging to
the Church, Baptism and Confirmation are intimately connected.
Consider choosing one of your Godparents as a Confirmation sponsor.
Talk with your parents about who can best support you during the
Confirmation preparation - as well as after. No your parents
cannot be your sponsor. At your Baptism, your Godparents represented
the community of faith in welcoming you to the family of the Church.
They and your parents accepted a responsibility at your
Baptism to help you grow as a Catholic. Our Church wants you to
journey with others in the community of faith, someone other than
your mom or dad.
